We welcome referrals from Local Authorities, NHS teams, Integrated Care Boards (ICBs), social workers, hospital discharge teams, case managers and other health and social care professionals.
We can support with assessments, planned care starts, hospital discharge, domiciliary care, Complex Care packages and ongoing reviews. Please contact our team to discuss an individual referral or service requirement.
